General Power Of Attorney Template

I decided to give power of attorney to my sister to take care of my daughter. I tried to find how. Mostly, I was told to look on the internet for free form, but I could not find one that provides custodial care of a child (without having to pay for one). I found those relating to other matters (for free), but not what I'm looking for. I finally realized I can write my own but I'm not sure how to go about one or what it should say. Does anyone have a template that I can use or offer suggestions as to what it should saya I know it would be better resolved with a lawyer, but I can not afford (or I would not be here) if anyone can help me, please do so.

The local department of social services should be able to assist you in this FREE.

In many states, it would be subject to approval by the competent court and child protection services section of the DSS.
