Legal name change (the husband) - Marriage certificate shows the surname old - Ontario, Canadaa My wife and I married in Cuba in 2006, I legally changed my last name (it was a hyphen - I removed the link to my new family =)). My wife has trouble changing his name on some of his ID because the marriage certificate proves again the old name. We can not get the marriage certificate changed (with my new name) because we were married outside of Canada. Does anyone know of a legal document / hyperlinks (Ontario Canada), which says it is legal to use an official "Change of name certificate" as a supporting document to the marriage if she (we) do not receive these worries when trying to update his health card, SIN card, etc.
Take the county courthouse near you. They should be able to fix everything.
Hello, I am currently the same problem with my social insurance card. My husband and I were married in Mexico and since then it changed legally changed his last name. I was told that I am not able to change my social insurance number to reflect its name. Made this solution work for you may Pera Flag
