No, you do not have one, but it would be a good idea. It will cost about $ 300.
As far as I found your name on your birth certificate is your name, according to South Carolina. Then you just submit a request for name change - the court clerk should have the form, do not know why they would be baffled by it.

A legal name change requires going through the procedures in your state - at least I checked need a court order.
Once you have the court order authorizing the name change, you simply send a copy of the order of the court in the state where your birth certificate is on file, and make them change.
