Am I obligated to complete this form stating that my doctor treat me if I suffer an injury at worka I'm starting a new job and had to fill out some paperwork. a newspaper is called the medical staff prenamed. Basically, he said if I suffer an injury or illness related to my job, my medical doctor to treat me. it says:
you can use this form to notify your employer if you want your personal medical doctor treating you for a work related injury or illness.

So here's the thing. Firstly, I do not expect to be injured on this job. I am basically a cashier in a store. Secondly, I can not say if this form is required or not. should I call my doctor, I have not even seen lately, and have them sign the forma Would it look bad to my new employer if I just go to the nexta Finally, I'm in New York, not California.
Nobody expects to get hurt at work, but sometimes even the cashiers. What does this form lets you designate your doctor that the doctor that you will use in the case of an accident at WC.
