Physician Employment Agreement

I'm starting a new job and had to fill out some paperwork. a newspaper is called the medical staff prenamed. Basically, he said if I suffer an injury or illness related to my job, my medical doctor to treat me. it says:

you can use this form to notify your employer if you want your personal medical doctor treating you for a work related injury or illness.
