Statutory Declaration Form NswWhat to write on a form for tattoos for minorsa

I get one week next tattoo in NSW, Australia. It is legal to tattoo a person over 16 as long as their parent signs a statutory declaration form.
I do not know what to write on the form in the part where it asks me to write what my mother is "declare".

Something like "I give my daughter permission to get a tattoo of blah blah to this place on his body by the tattoo artist" or something like that, but is there a generic statement that I can usea I not want there to be flaws, like I'm going to a place a little way from me so I can be sure it is reputable, and I'm not going with my mother if we should go back and change the form around I'll be very disappointed.

What about "I give my daughter, (your name) to get a tattoo at (place you get to) and understand the risks involved."

If you're still not sure call them again and ask exactly what to say.

They could not even do that unless your mother fills in fact part of the form. Here in the States, a minor can not sign a form from their parent, the parent must be present, so I guess it's the same thing there. I would not waste my time or money on the trip there if I were you, without trying to know if the parent must complete this section of the form themselves.

Additionally, if your mother did not say it was ok, and you do it behind his back (not saying you are, just hypothetically), you create 2 problems, 1. be you to be decreasing levels of trust with your mother significantly (never a good thing for a teenager), and 2. your mother could possibly take legal action against the studio / artist to do the tattoo without consent. Something to think about.
